It comes controls on the sides of the chair. Manufacturer: Isell Diversatronics Model: C4-H Serial Number: H41095070L The Exam Chair lifts and lowers properly. The manual Chair tilt works properly. The Chair does not come with a foot controller. The metal on one of the headrest pieces is cracked and will need to be fixed or replaced. The Model on the Chair base reads "C4-H", the model on the sticker label is "C 4 L". The seat upholstery has a tear in it.
